ObjectiveHead and neck cancer treatments are known to significantly affect functionality and appearance, leading to an increased risk for body image disturbances. Yet, few longitudinal studies exist to examine body image in these patients. Based on a conceptual model, the current study aimed to determine, in patients newly diagnosed with HNC: the prevalence, level, and course of body image concerns; correlates of upon cancer diagnosis body image concerns; predictors of immediate post-treatment body image concerns; and association between body image concerns (...) and levels of anxiety, depression, suicidal ideation, support, and alcohol and drug misuse.MethodsTwo hundred and twenty-three, newly diagnosed with a primary HNC were assessed using structured clinical interviews and psychometric measures at three, and 6 months after diagnosis. Primary outcome was 3-month, as it was most salient to body image disturbance. Multiple linear regression analyses were conducted on the potential body image predictors, based on the model.ResultsSixty-eight percent of patients with HNC presented some level of body image concerns. Body image concerns at baseline and post-treatment were significantly related and significantly increased from pre- to post-treatment. Immediately post-treatment, 89% presented some level of body image concerns. Correlates of body image concerns in patients with HNC at baseline included: physical symptom burden, difficulties with communication and eating, coping with the cancer diagnosis using denial, suicidal ideation, and having had a past anxiety diagnosis. When controlling for sociodemographic and medical variables, body image concerns in patients with HNC in the immediate post-treatment were predicted by: baseline body image, physical symptom burden, and neuroticism.ConclusionThis longitudinal study helps identify patients more susceptible to experience body image disturbance following head and neck cancer. Clinicians ought to pay special attention to body image concerns upon cancer diagnosis, physical symptom burden, and neuroticism, and may want to target these factors in future preventive interventions. L'article retrace l'histoire de l'image du peuple de Dieu comme brebis et pasteur dans l'Ecriture, dans deux figures représentatives de l'époque patristique, Augustin d'Hyppone et Grégoire de Nazianze, ainsi que dans la Constitution Dogmatique sur l'Eglise dans le Second Concile du Vatican. L'article fait une application précise de cette image au phénomène contemporain d'un changement culturel rapide dans le monde et dans l'Eglise. L'article soutient que l'image biblique des brebis et du pasteur ne doit pas être abandonnée comme un pûr (...) artifice culturel qui reflète une structure sociale primitive désormais entièrement désuète. Plutôt, cette image est une affirmation vitale et importante, tant anthropologique que ecclésiologique, sur la nature humaine, le besoin humain de communauté, d'autorité, de direction et d'attention. Il est question de l'analogie et de la comparaison selon la démarche dialectique platonicienne, ainsi que des topoi empruntés aux textes anciens: la chasse, le labyrinthe, le vase, la statue intérieure que l'on doit polir, le Soleil pour désigner de manière visible le Bien, ou la caverne pour rappeler la parabole de laconnaissance, mais aussi pour évoquer le lieu de transit de l'âme. Les principaux textes utilisés sont extraits des Discours théologiques de Grégoire de Nazianze. Un passage du Traité sur (...) le Saint Esprit (149 B-C) de Basile de Césarée, cité à la fin, illustre dans un contexte un peu différent le même procédé que celui utilisé par Grégoire. Dans tous ces passages il est question de travailler le langage sous le double aspect ontologique et sémantique, afin de trouver les modalités discursives les plus appropriées à la définition de la substance divine, à la nature de l'unité divine et à la détermination du caractère spécifique des hypostases. La théologie chrétienne se sert donc des moyens de la philosophie et emprunte des figures à la rhétorique, mais se défend de toute confusion avec ces disciplines. Invente-t-elle alors un genre nouveau, le «discours théologique», sur le modèle fourni par les Cappadociens? Encore faut-il savoir si le discours pouvait être perçu au IVᵉ siècle comme un genre approprié à la théologie, ou seulement comme un contrepoint à l'exégèse et au traité: un contrepoint par lequel les Pères ne rechignent pas à faire concurrence aux Sophistes pour les besoins politiques, et subsidiairement polémiques, de l'institution nouvellement créée. Psychoactive drugs are being prescribed to millions of Americans at an increasing rate. In many cases these drugs are necessary in order to overcome debilitating emotional problems. Yet in other instances, these drugs are used to supplant, not supplement, interpersonal therapy. The process of overcoming emotional obstacles by introspection and the attainment of self knowledge is gradually being eroded via the gratuitous use of psychoactive medication in order to rapidly attain a release from the common problems that life inevitably presents (...) us with. In this paper, I argue that Kant’s formula of humanity, which maintains that persons ought never to treat others or themselves soley as a means to an end, proscribes this. Moreover, Kant argues that we have an imperfect duty of self development, and I argue that we fail to adhere to such a duty whenever we seek to evade the process of introspection and self knowledge in favour of the expedient results that drugs may provide us with as we attempt to overcome the emotional hurdles in our lives. This paper describes the perspectives of stakeholders within the North American dairy industry on key issues affecting the welfare of dairy cattle. Five heterogeneous focus groups were held during a dairy cattle welfare meeting in Guelph, Canada in October 2012. Each group contained between 7 and 10 participants and consisted of a mix of dairy producers, veterinarians, academics, students, and dairy industry specialists. The 1-h facilitated discussions were audio-recorded and transcribed verbatim. Content analysis of the resulting transcripts showed that participants (...) across all stakeholder categories identified lameness as the most important welfare issue facing dairy cattle. Other prominent issues of concern included cow comfort, disease, on-farm mortality, stockmanship, painful procedures, injuries, cull cow management, calf management, and restriction of behavioural freedoms. Participants typically gave several reasons for why they considered issues problematic. Underlying reasons were grouped according to animal-centered concerns [at both the individual and at the herd level ] and industry-centered concerns . This analysis identified areas of shared concern among diverse stakeholder groups, which should aid in the development of standards and policies that satisfy stakeholders within and external to the dairy industry. Objectives: To compare attitudes of medical and law students toward informing a cancer patient about diagnosis and prognosis and to examine whether differences are related to different convictions about benefit or harm of information.Setting and design: Anonymous questionnaires were distributed to convenience samples of students at the University of Geneva containing four vignettes describing a cancer patient who wishes, or alternatively, who does not wish to be told the truth.Participants: One hundred and twenty seven medical students and 168 law students.Main (...) outcome measures: Five point Likert scale of responses to the vignettes ranging from “certainly inform” to “certainly not inform” the patient.Results: All medical students and 96% of law students favoured information about the diagnosis of cancer if the patient requests it. Seventy four per cent of medical students and 82% of law students favoured informing a cancer patient about his or her prognosis . Thirty five per cent of law students and 11.7% of medical students favoured telling about the diagnosis and 25.6% of law students and 7% of medical students favoured telling about the prognosis even if the patient had clearly expressed his wish not to be informed. Law students indicated significantly more often than medical students reasons to do with the patient’s good, legal obligations, and the physician’s obligation to tell the truth, and significantly less often than medical students that their attitude had been determined predominantly by respect for the autonomous choice of the patient.Conclusion: Differences in attitudes according to the type of case and the type of studies were related to convictions about the benefit or harm to the patient caused by being given information. The self reported reasons of future physicians and future lawyers are helpful when considering means to achieve a better acceptance of patients’ right to know and not to know. (shrink)

In our opinion, the flaws in the teaching of philosophy were to a large degree related to the fact that it was regarded as a science . The resolute rejection of this position and the recognition of philosophy's status as a world view is for us the necessary condition for escaping from the existing situation. To put it more concretely, the view of philosophy that became entrenched among us has produced the following fundamental flaws in the teaching process:—An underestimation of (...) or even a dismissive attitude toward pre- Marxist and in particular non-Marxist theories . Although non-Marxist philosophy was looked at in the course of study, this was done mainly to show its shortcomings—to the greater glory of Marxist philosophy. Idealism was regarded only as a barren flower. Students were instilled with the idea that idealism was on the verge of being overcome and supplanted by materialism, and this created in them a completely untrue picture of the real course of development of philosophical knowledge. The paper focuses on the problematic of selfinterpretation and the correspondence between selfimage and the reality. Its presupposition is, that a human being is not committed to a single way of life, fully determined by his/her biological characteristics or by the dictate of his/her social background; first of all, he/she is not dependent on single ways of the interpretation of the world or of his/her selfinterpretation. It is his/her ability to accept certain characteristics as his/her own that makes him/her a (...) human being. There is usually a distinction between his/her own seeing himself/herself, which does not necessarily, leads to a failure. The strategies of survival are various and they need not to be rooted in true interpretations in all their aspects. In conclusion the author argues, that the critical thinking making its way into everyday life as well as into the public sphere offers the possibility of the variability of the ways of life and of a consensual coexistence on the basis of manifold interpretative structures and models. Enforcing a uniform view as a presupposition of human cooperation is replaced by an approach, respecting the legitimacy of the pluralistic image of the world and of the humans, for which this approach makes no obstacle in cooperative working of society. Philosophy of history is analyzed here from the point of its epistemological and ontological meaning. The author considers that the ontological point of view makes it possible to conceptualize the history as the unity of its all times - Past, Present and Future. The connection between these three times based on their relation to the concept of Eternity which has been symbolically formed within the mythological, religious and utopian Weltanschauung. The necessity of these relations transforms philosophy of history into a (...) special kind of ontological historical knowledge. The study of history from the prospect of unity is only possible within the historical interpretation of Future as an epoch of bridging the gap between Time and Eternity. This article presents a brief general view of the recent literature and the scholarly activity in the field of philosophy of medicine in Scandinavia. The focus of attention is not on medical ethics, but on studies on topics like decision theory, medical classification, causality, causal explanations, concept formation, and on analyses of different ideals of medical science and clinical practice. A few principal works on medical ethics are mentioned by way of introduction and a brief account of a highly topical (...) debate on the legislation on artificial insemination in Sweden is given at the end.
